DISCLAIMER STATEMENT FOR CLIENTS OF GLO TAN 

  • Please read this disclaimer carefully and if you do not agree with it, please ensure you do not use our product and please do not carry out your service with us.
  • All ingredients in this product are intended for cosmetic use and generally regarded as safe. There are, however, occasions where individuals may be allergic to one of more of the ingredients in the spray tan solution.
  • You are responsible for your own due diligence of the product prior to use.
  • If you have any medical conditions or allergies please do not use our product and refrain from going through with your spray tan service. 
  • It is your responsibility to make full disclosure to us in advance of any allergies, concerns or medical conditions.
  • Glotan waives any responsibility for any liability that may occur due to the use of our product and in going through with spray tan service, you do so of your own accord and after completing your own due diligence of the product.
  • In the event you proceed with a spray tan service with Glotan, You further confirm that you have no concerns, allergies or medical conditions that prevent you from using our product and you do so of your own free will and volition.
  • If you are pregnant or nursing we always recommend you consult with your doctor or physician before receiving a spray tan.
  • WARNING- This product does not contain a sunscreen and does not protect against sunburn.

DISCLAIMER STATEMENT FOR MODELS IN PHOTO AND VIDEO SHOOTS WITH GLOTAN

Please read this disclaimer carefully and if you do not agree with it, please ensure you do not use our product and please do not carry out any modelling photo or video shoots with us. 

  • All ingredients in our products are intended for cosmetic use and generally regarded as safe.
  • There are however occasions where individuals may be allergic or react to one or more of the ingredients in the solution.
  • You are responsible for your own due diligence of the product prior to use.
  • If you have any medical conditions or allergies please do not use our product and refrain from going through with the modelling photo or video shoot with us.
  • It is your responsibility to make full disclosure to us in advance of any allergies, concerns or medical conditions.
  • Glotan waves any responsibility for any liability that may occur due to the use of our product and in going through with the photo or video shoot with us, you do so of your own accord and after completing your own due diligence of the product.
  • In the event you proceed with a modelling photo or video shoot with Glotan, you understand and agree that this service is complimentary for you without any royalties or fees owing to you and that we reserve the rights to use the photos and videos, unconditionally.
  • You further confirm that you have no concerns, allergies or medical conditions that prevent you from using our product and you do so of your own free will and volition.
  • If you proceed with any photo or video shoot with Glotan, we will assume your full consent to this disclaimer and in doing so, you waive all claims against Glotan.
  • If you are pregnant or nursing we always recommend you consult with your doctor or physician before receiving a spray tan.

Consent and Disclaimer

I hereby consent and authorise inQ Digital LLC (GLOTAN) to provide me with ‘spray tan’, subject to the information I have disclosed in this form. If, for any reason, any aspect of my disclosure herein proves to be inaccurate, incomplete or incorrect or I do not adhere to my undertakings or breach my warranties hereunder, I understand it is my sole responsibility and I will not hold GLOTAN liable nor make any claim for any adverse reactions or effects or no reactions or no effects.

 I further confirm the following:

  1. I do not have any medical conditions, allergies, health disorders, or problems that would prevent me from receiving a spray tan;

  2. I am currently pregnant or breastfeeding and have sought consent from my doctor to proceed with the spray tan process;

  3. I am not taking any medications that would impede me getting a sunless spray tan;

  4. I have not had any surgical or aesthetical procedures in the last month; 

  5. I have not had any laser treatments, hair removal or skin bleaching treatment in the past 7 days;

  6. I consent to the terms and conditions available at https://glotandxb.com/terms-and-conditions/ 

 Bookings and Cancellations

I agree to the following booking and cancellations policy:

(i)  I will provide not less than twenty (24) hours notice for cancellation of any session. If an appointment is cancelled less than 24 hours prior to an appointment, I will be charged 50% of the appointment fee.

(ii) Appointments shall be at the scheduled time. 

(iii) If I arrive late to an appointment, I may need to wait or be rescheduled to an alternative available time.

(iii) If I do not show up to a booking, the full appointment fee is forfeited or becomes payable in full.

(iv) GLOTAN reserves the right to cancel an appointment at its sole discretion in which case I will not be charged and any sums paid will  be returned in such case.

 Payments

I agree to the following payments policy:

(i) I agree to pay all amounts agreed to GLOTAN in cleared funds without delay, deduction, set-off or discount.

(ii) I understand GLOTAN does not provide refunds, barters, compensation or transfers of my bookings or advance payments (if any). Payments are fixed, non-negotiable, non-refundable and non-transferrable.

(iii) More than one promotion or discount may not be combined.

(iv) Vouchers may only be used during their validity period and cannot be extended, redeemed for cash/products or transferred to another party.

(v)  Glo Tan reserves the right to change the prices of their products and services at any time.

 Acknowledgment

I understand and agree:

  1. All ingredients in GLOTAN’s spray tan product are intended for cosmetic use and generally regarded as safe. There are, however, occasions where individuals may be allergic to one of more of the ingredients in the spray tan solution. If this occurs I understand that I should immediately discontinue use. There are a small number of individuals whose skin does not react favorably to spray tanning. Medications such as birth control, changes in hormones or hormone medication and antibiotics may alter the results of a spray tan.

  2. This spray tan product does not contain a sunscreen and does not protect against sunburn.

Personal Data

I understand that my personal data is stored with you securely and not shared with any third party nor used for any marketing or promotional purposes. GLOTAN can however keep me updated on promotions if I so wish.

 Promotional Videos and Marketing Material

I fully understand and agree that any promotional videos or marketing material that I may be shown or come across with respect to the product or services provided by GLOTAN are purely for marketing and promotional purposes and GLOTAN makes no claims or warranties nor provides any guarantees or undertakings to me. I know that I am responsible to undertake my own research with respect to the spray tan process and understanding the spray tan product. 

 Release and Discharge

I hereby release, discharge, waive and hold harmless, permanently and fully, GLOTAN and all its shareholders, directors, employees, representatives and staff members against all claims, losses, damages or losses I may incur or sustain in undertaking the process stated herein.

 Indemnity

I hereby fully indemnify, hold harmless and release GLOTAN, its shareholders, directors, managers, employees and representatives from any claims, losses, damages or unless caused fraudulently or with malicious intent in which case GLOTAN’s liability if proven in all cases is limited to the value of the fees paid to GLOTAN with respect to the claimed session. 

 Client Undertakings and Warranties

By booking, I warrant and undertake to GLOTAN as follows:

  1. I am of legal age (above 21 years). In the event that I am below twenty one (21) years of age this form will be signed also by my parent / legal guardian as their full consensus to its terms;

  2. I have fully read, understand and agree with these terms;

  3. All the information I have disclosed herein is true, accurate and complete;

  4. I have been offered eye goggles and nose blockers and understand and agree that if I do not use these safety guards, I do so at my own risk;

  5. I have been given adequate instructions for the proper use of the sunless tanning application, understand the risks involved, and use it at my own risk;

  6. I fully understand and have undertaken my own due diligence into the sunless tanning product and service offered by GLOTAN and am undergoing the process with GLOTAN, in full and clear understanding, of my own will and volition;

  7. I shall not distract our application personnel during the spray tan sessions nor permit third parties to enter the application areas;

  8. GLOTAN does not provide me with any medication or medical advice and if I require such, I will seek out a registered and licensed medical practitioner.

 Governing Law and Dispute Resolution

I agree that the relationship between myself and GLOTAN is governed by the laws of the United Arab Emirates. I further confirm and agree that if I have any claims that are proven directly to be arising due to breach by GLOTAN and (subject to my full compliance with the terms hereunder), I agree that I may bring any claims exclusively through arbitration by a single arbitrator in the English language with its seat in Dubai, UAE under the rules of the Dubai International Arbitration Centre.

Acceptance 

When you complete your booking, you accept these Terms and Conditions  and any other ones that you‘re provided with during the tanning service
